A parrot, cockatiel or canary that suddenly stops perching, struggles to breathe or refuses food for a whole day did not become ill at that moment: it became ill earlier, often much earlier, and simply lost the ability to mask it any longer. This is the single most important biological fact for anyone keeping a bird at home, and it is also the most counterintuitive for someone coming from dogs or cats: in companion mammals discomfort tends to show up early through signs that are easy to read, while in birds a visible sign almost always arrives at an already advanced stage of the problem. Why a bird hides being unwell

Within a flock or a social group, the individual that shows weakness is the first target for a predator and is often also the first to be excluded or attacked by the rest of the group. Natural selection therefore favoured, over time, individuals able to keep behaving normally, upright posture, tight plumage, apparent activity, even while a disease process is already underway internally. That instinct does not disappear in a cage: a bird kept indoors for generations still carries the same behavioural repertoire as its wild ancestors.
The practical consequence is that the moment an owner notices a clear symptom, laboured breathing, persistently fluffed feathers, sitting motionless on the cage floor, almost always coincides with the animal running out of its ability to compensate, not with the start of the problem. A healthy bird keeps its plumage sleek against the body for most of the day and only fluffs it up briefly, for instance while resting or in a cooler than usual environment. Open mouth breathing, on the other hand, is almost always a later and more serious sign: birds normally breathe through their nares, and holding the beak open to breathe, especially at rest and not right after a fright or exertion, indicates already significant respiratory difficulty. Also watch for unusual sounds during breathing, clicks, wheezes, audible snaps at close range, and for changes in song or usual vocalisation: a bird that stops singing, sings in a different voice than usual, or becomes unusually quiet or unusually loud compared with its own norm is communicating something.

Droppings: the three parts worth knowing
A healthy bird's droppings are made of three recognisable parts once you know what to look for: a more solid faecal portion, generally dark or greenish brown, a white or off white creamy urate portion, and a liquid component, the actual urine, which is normally present only in a small amount. Each of the three parts can change independently of the others, and that is exactly why it is useful to look at them separately instead of judging the droppings as a single whole.
Changes in colour, consistency or the proportion between the three parts compared with what that bird normally produces deserve attention: a liquid portion that stays consistently increased, a faecal portion that changes colour or consistency for more than a day, or the appearance of blood in any of the three parts are all reasons to contact the vet. It is worth remembering that some foods, particularly brightly coloured fruit and vegetables, can temporarily change the colour of droppings without indicating a problem: the useful criterion is always comparison with that individual's own norm over time, not a single isolated episode.

A sheet of white paper or kitchen paper on the cage floor makes it far easier to tell the three parts apart and to notice colour changes compared with a granular litter.

A drop in actual intake, a change in usual food preferences, or abandoning a food the bird always readily took are signals worth noting. The same applies to overall activity level through the day and to moulting: a moult that starts outside the usual seasonal pattern for that individual, that runs unusually long, or that produces irregular bald patches instead of the normal orderly feather replacement, deserves evaluation.
For all of these signals, the right reference point is a veterinarian with specific training in exotic or avian medicine, not a generic small animal vet: bird physiology is different enough from that of dogs and cats to require dedicated expertise, both for interpreting signs and for the clinical exam and any diagnostics needed. Identifying an avian vet in your area in advance, when possible before you actually need one, avoids having to search for one under pressure in the middle of an emergency.
A few environmental risks deserve a separate mention because they can be lethal very quickly and are easily avoided once known.
